But you can still go back to the content editing tab and carry on using the CMS as normal. Once this has happened, all previews result in this page shown above (or a 503).
The only way to fix this for the client is they logout and back into the CMS
No, although we were able to narrow it down to the Application Gateway in Azure.
When used without the AG, the CMS works fine, but via the AG, we get this issue.

We have had a strange issue on a live site.

I'm having similar issues.. no 503 just a white page and unable to load 2 core Umbraco files. Did you ever get a solution?
