After upgrading Umbraco Forms to 4.4.0 I started to get a JS error that the forms admin couldn't find the setting JSON data and I could not edit the settings for the default Umbraco Form fields Perplex field work fine. Forms rendered on the front end okay as well. I upgraded to version 1.55 of the extension to see if that would fix the issue but it did not. So after a bit on investigation I notice that all of the default Umbraco Forms Fields were now missing in the admin. I tested on a fresh install of Umbraco Forms and before installing the Perplex plugin the field were available to add to the form but after only the Perplex fields were available. Because the default fields are missing forms that had default Umbraco Form field are no long editable. Any help would be appreciated.
Update: Did some more testing and found that the issue that I am having is not an Umbraco forms issue. I rolled back Umbraco Forms to 4.3.3 and got the same issue with the default form fields missing. I rolled the Perplex plugin back to v1.32 and all was good again. Upgrade Umbraco Forms back to the latest version, 4.4.0 and all was still good and all expected forms field were again available. So the issue seems to be with v1.55 and I suspect 1.5 as well but I did not test that exact version.
Also looks like the Perplex plugin does not work with the file system provider functionality new in Umbraco Forms 4.4.0.
We introduced some configuration options in version 1.55, which includes the ability to hide certain field types. By default we hide the Umbraco field types for which we provide an alternative version. You can change this in \App_Plugins\PerplexUmbracoForms\PerplexUmbracoForms.config (simply change hide="true" to hide="false" for the Umbraco field types).
With regards to the file system provider functionality; you are right. We have noticed that uploaded files are not stored anymore due to this change in Umbraco Forms. I hope we can upload a new version soon (this week or otherwise it might be January).

I just uploaded a new version (1.6) which should fix the compatibility issues with Forms 4.4. Uploaded files should save properly now.
